The Accident Investigation Board Norway (AIBN), the Defence Accident Investigation Board Norway (DAIBN) together with the Marine Safety Investigation Unit of Malta and the Spanish Standing Commission for Maritime Accident and Incident Investigations (CIAIM) have conducted a joint investigation for the collision of the frigate HNoMS Helge Ingstad and the oil tanker Sola TS.

The National Transportation Safety Board (NTSB) issued an accident report according to which on December 30, 2018, the passenger vessel Nippon Maru struck mooring dolphins at a US Navy fuelling wharf in Apra Harbor, Guam, revealing that the master had consumed alcohol before and after the accident. 

The National Transportation Safety Board issued an accident report concerning the cruise ship "Carnival Horizon", which in attempts to maneuvre its entrance into berth no. 2 at Manhattan Cruise Terminal’s Pier 88 in New York City, New York, its bow struck the southwest corner of adjacent Pier 90.

The National Transportation Safety Board launched an accident report focusing on the sinking of  the towing vessel Jacob Kyle Rusthoven which caught fire prior to being entirely lost in the sea; The Safety Board suggests that what caused the fire was an engine lube oil leak which ignited off a hot surface near the starboard main engine turbocharger.

The US National Transportation Safety Board published its report on the liftboat Ram XVIII, that overturned in the Gulf of Mexico, on November 18. Due to the incident, three personnel suffered minor injuries, while about 1,000 gallons of hydraulic oil were released. The vessel was declared a constructive total loss at an estimated $1,140,000.

In its latest Safety Digest, UK MAIB focuses on how a mooring line from a ferry, can easily be tangled in the propeller, without the right communication between the ship personnel and the shore. Based on this accident, MAIB advises to always follow the directions of the safety system and keep in mind that routine operations allow safe practices to be tested.

UK MAIB has published its report on the UK registered charter yacht, Tyger of London, whose keel failed suddenly on 7 December 2017 and the yacht capsized. The crew, all wearing lifejackets, were thrown into the water and rescued by the British Army yacht, St Barbara V.

The Australian Transport Safety Bureau (ATSB) launched a report about the injury of two crewmen on a bulk carrier, highlighting the importance of following procedures and applying best practice for safer operations.

New Zealand's TAIC issued an investigation report on the grounding of the Danish-registered container ship Leda Maersk off the Port of Otago in June 2018. The investigation revealed that neither the harbour pilot nor the ship’s bridge team recognised that the Leda Maersk was deviating from the planned track. 

RMI issued an investigation report on a fatal fall onboard the bulk carrier 'Penelope' while underway in the Coral Sea in November 2018. A seafarer was standing on the extension platform for one of the ship’s provision cranes which detached and fell 8-9 meters.
