UK MAIB published its report on a man overboard incident from the single-handed creel boat Sea Mist. The incident took place on 27 March 2019, and resulted to the loss of one life.

UK MAIB issued an accident report on a fatal incident, when a skipper became entangled in a back rope while shooting creels and was hauled overboard; The son of the ill-fated skipper saw the  single-handed creel boat Sea Mist wandering around without him on board and raised the alarm.

The Australian Transport Safety Bureau (ATSB) issued an investigation report on the grounding of the bulk carrier 'Orient Centaur', which lost power while transiting a channel from the Port of Weipa, in November 2017. This incident highlighted gaps in the port’s risk plans, ATSB noted. 

The Accident Investigation Board of Norway issued its accident report, focusing on the Norwegian cruise vessel "Viking Sky", which engines failed during a storm in the Norwegian sea, resulting to the vessel drifting in rough waters in the Norwegian Sea to within 100 meters of land.

Based on fatal incident of the DUKW Amphibious Vessel, Stretch Duck 7, the NTSB issued Marine Safety Recommendation Report, calling the USCG to require sufficient buoyancy for DUKW amphibious passenger vessels and to require the removal of canopies, side curtains and their associated framing, while underway, for those without sufficient reserve buoyancy.

The Japan Transport Safety Board issued a report concerning the collision of the 25,431-ton cargo vessel ERNA OLDENDORFF, with the Oshima Bridge, in Obatake Seto, Yamaguchi Prefecture, Japan, during the early hours of Monday, October 22, 2018.

The Republic of the Marshall Islands in its latest accident report focuses on the fatal injury of an oil tanker electrician, who got electrocuted on board "STI WINNIE" vessel. Investigators of the incident, assumed that he accidentally touched the live 440 volt (V) power terminals in the cabinet’s lower section, which led to his death.

Transport Malta's MSIU issued an investigation report on the fatal fall of the second engineer onboard the general cargo ship MV Khosrov Bey while underway in November 2018. The investigation noted the high level of alcohol may have impaired his cognition or caused poor coordination. 

Transport Malta's MSIU issued an investigation report on a fatality and another serious injury of two crew members onboard the Maltese registered bulk carrier MV Balgarka, while involved in repairs in dry-docking at the Bulyard Shipyard, Bulgaria, in October 2018. 

Transport Malta issued an accident report focusing on a vessel which faced many challenges navigating through rough weather conditions; Although all works were suspended in sight of the heavy weather, equipment in the engine room broke and fatally injured two crewmembers.
