UK MAIB recently published its investigation report, focusing on a roll-on/roll-off (ro-ro) passenger vessel which rolled heavily due to very rough seas and winding conditions. Namely, ferry's cargo sifted and caused damage to the vehicles onboard.

In fact, the UK MAIB recently issued its investigation report for the accident on the then UK-flagged container ship CMA CGM G. Washington while it was experiencing very heavy seas in the North Pacific Ocean during its passage from Xiamen, China to Los Angeles, USA.

The NTSB issued an investigation report on the capsizing and sinking of Barge Dredge200 and a workboat while on Massachusetts Bay, in December 2018. The accident was a result of a poor judgement amid inclement weather conditions. 

Transport Malta's MSIU issued an investigation report on a fatal fall of a crew member of the Maltese-registered bulk carrier 'New Island' in Greek waters, in December 2018. While in the cargo hold, the AB fell from a height of approximately six to seven meters onto the tank top.

While anchored in Yokohama, Keihin Port, a cargo ship dragged the anchor, drifted toward to the northeast, and collided with the seawall at Ogishima, Kawasaki section. Typhoon No. 24 was approaching. The Japan Transport Safety Board issued an investigation report on the incident. 

The NTSB issued an investigation report on the contact of the Andrew Cargill MacMillan towing vessel with a conveyor at the Farmers Grain Terminal, while pushing 42 loaded barges on the Lower Mississippi River, near Tallulah, Louisiana. The key cause identified was the pilot’s over-reliance on floating aids to navigation. 

The UK MAIB issued an interim report concerning an explosion and fire on board the chemical tanker Stolt Groenland which occurred that day in Ulsan, Republic of Korea, looking for the factors that caused the explosion.

NTSB issued an investigation report on the explosion and fire onboard the offshore supply vessel 'Grand Sun' while in the Gulf of Mexico, in October 2018. The report identified overheating of electrical wiring, associated with a chest freezer or the receptacle powering it, as key cause. 

The Federal Bureau of Maritime Casualty issued an investigation report on vessel "Ventura", to look for the causes that resulted to the vessel's cargo tanks and cargo being burnt down, highlighting that sugar beep pulp is prone to ignition when moistured and heaped. 

NTSB published its report on the 2018 grounding of a fishing vessel near Point Reyes, California. The five crew of the ship remained on the vessel, until receiving assistance from the USCG. There were no pollution or injuries.
