Hong Kong MARDEP shared the findings of a serious fire accident, which took place onboard the accommodation of a Hong Kong registered bulk carrier whilst at anchorage. The investigation showed that the fire was most probably caused by a faulty electrical device or its accessories in a crew cabin which led to a short circuit with the heat and sparks igniting the materials placed beside it.

At the beginning of January 2021, RightShip and INTERCARGO launched DryBMS which is a new quality standard for the dry bulk sector. The standard will be governed by a new NGO to be established later this year and will support the improvement of safety in the dry bulk segment. 

The US Coast Guard is evaluating the potential use of Safety Management Systems (SMS) to improve safety and reduce marine casualties onboard US-flagged passenger vessels.

An AB onboard a product tanker suffered severe injuries after falling from a portable ladder, while conducting maintenance work on the launching system for the ship’s free-fall lifeboat and later tragically died from his injuries. The Britannia P&I Club described this case study as part of its BSafe campaign. 

Failure to ensure cyber risk management is appropriately addressed in SMS by the company's first annual verification of the Document of Compliance after January 1, 2021, may result in detention of a ship in US port, BIMCO warned. 

The US Coast Guard Office of Commercial Vessel Compliance issued cyber risk management guidance for USCG Marine Inspectors and PSC Officers. Cyber risk management must be implemented into vessel SMS by the first ISM Document of Compliance verification after January 1, 2021.

In the new era of digitalization, the ISM Code, supported by the IMO Resolution MSC.428(98), requires ship owners and managers to assess cyber risk and implement relevant measures across all functions of their safety management system.

RMI issued an investigation report on a bosun's fatal fall overboard from the general cargo ship DELICATA, off Uruguay, in March 2020. The investigation identified poor compliance with the company's SMS and safe working practices, as the crew member was not wearing a lifejacket or a safety harness when he descended to untie a pilot ladder. 

Transport Malta MSIU issued an investigation report on the allision of the bulk carrier MV KLIMA with the Marshall Island-flagged bulk carrier SPRING SNOW and the Singapore-registered bunker barge LIBRA in the Eastern Special Purpose Anchorage, Singapore, in July 2019.

RMI issued an investigation report on a fatal fall onboard the container ship AL RAWDAH in January 2019. The report stressed issues associated with poor SMS implementation and lack of safety harness.
