According to the Brazilian oil workers’ union Sindipetro-NF, an offshore oil worker died on the PNA-2 platform, on November 25. The accident took place while the worker was servicing a crane. On Sunday afternoon, the worker was killed by the accident that occurred on PNA-2 platform while he was operating a crane and was crushed by the equipment.

At least 31 people have lost their lives and more than 50 are still unaccounted for, after a boat capsized on Lake Victoria in Uganda, Saturday, according to data provided by the Uganda Police Force. The police said that the key cause of the accident was over-loading and the vessel's poor mechanical shape.

Transport Malta issued a report on a fatal occupational accident onboard 'Eleni M', as the crew member got trapped between the mooring rope on the winch drum and the deck on the poop deck. The investigation revealed that in order to perform the task of stowing the mooring ropes on the winch drum, the crew had to engage physically with the ropes while the winch was rotating.

EMSA released its annual review of maritime casualties reporting a total of 3,301 occurrences through 2017. The report reveals that the number of very serious casualties has continuously decreased since 2014 with 74 reported in 2017. A total of 61 people were injured and 61 ships were lost. During the 2011-2017 period, 405 accidents led to a total of 683 lives lost, which represents a significant decrease since 2015.

A seafarer was killed while lashing and securing containers onboard the Antigua and Barbuda-flagged container ship 'Francop', in the port of Dublin, Ireland, on 14 November. Dennis Gomez Regana, from the Philippines, died after a container struck him at Southbank Quay, when he was doing lashing work while container lifting operations were underway, ITF informed. 

Ireland's Marine Casualty Investigation Board issued an accident report on a fatal incident onboard the RoRo ferry 'European Endeavour'. While the ferry was loading the vehicular cargo, on 22nd June 2017, in Dublin, one of the crew was found unconscious at the rear of a trailer. After unsuccessful resuscitation attempts, he was declared dead.

Two dockers died on November 13, while they were unloading logs from a bulker in Montevideo, Uruguay. According to reports, two others were also transported to the hospital. Information reports that the victims were probably exposed to a fumigant. The authorities are further investigating to determine the cause of the exposure. 

Three crewmen onboard the general cargo ship 'Appolo Kita' have reportedly lost their lives from asphyxiation after an enclosed space entry, while the ship was was underway off Ishigakijima, on 9 November. The ship was en route from Kalimantan to Osaka, Japan, carrying wood, when the three crew members lost consciousness after entering a cargo hold.

The captain of the duck boat that sank on a Missouri lake during a storm in July, killing 17 people, was charged with misconduct, negligence and inattention to duty by a federal grand jury. The captain was charged in a 17-count indictment, one count for each of the passengers that died when the vessel sank.

Seventeen migrants have lost their lives this week, during their attempt to cross the sea from North Africa to Spain. In addition, rescuers have picked up over 100 migrants, the Spanish coast guard informed. Namely, 13 died between the Iberian peninsula and Morocco and Algeria, while the coast guard located another four bodies off Cadiz.
