Ireland's MCIB issued a report on a fatal sinking involving the fishing vessel 'Beal Sruthan' while off Craugh Island, Galway Bay, in May 2018. Although the cause cannot be determined, it is probable that the failure to properly blank off the redundant stern tube by failing to properly seal both ends led to the sinking. 

Transport Malta's MSIU issued a report on the fatality of two crew members onboard the chemical tanker 'Key Fighter' in September 2018. The two crew men fell from a height which led to fatal injuries. It was not excluded that the cause of death could have been either H2S gas intoxication or suffocation due to lack of O2.

Transport Malta issued a report on a garage fire onboard the yacht 'MY Kanga' while at anchor off Croatia, in September 2018. The investigation concluded that in all probability, the seat of the fire was the Lithium-ion batteries. Transport Malta recommended a review of the Commercial Yacht Code of 2015.

TSB issued an investigation report on a possible bottom contact involving the bulk carrier 'Maccoa' in Kahnawake, in November 2018. TSB noted that when a Seaway lift bridge cannot be raised to allow the vessel to pass, the vessel’s master must take appropriate action to avoid passing the limit of approach sign. 

Canada's Transportation Safety Board (TSB) issued an investigation report on the Captain fatality onboard the bulk carrier 'Sage Amazon' while at the Port-Daniel–Gascons, in Quebec, in March 2018. The Captain's death was caused by a cardiac event that led to his fall from ladder. 

NTSB issued an investigation report on the capsizing of the fish tender 'Pacific Knight' off Alaska in July 2018. The investigation identified several contributing factors to the capsizing, including poor stability assessment by the captain, vessel modifications, overloading and slack water in tanks. 

The National Transportation Safety Board issued an investigation report on the collision of two fishing vessels and the subsequent sinking of one vessel in the Gulf of Mexico in July 2018. The report identified issues related to poor lookout and inappropriate actions to avoid the collision. 

The National Transportation Safety Board issued an investigation report on the fire and subsequent sinking of the fishing vessel 'Master D' in the Gulf of Mexico in August 2018. The report identified leaking lube oil from the diesel generator as key cause of the fire. 

New Zealand's Transport Accident Investigation Commission released its report on the grounding of the passenger catamaran Dolphin Seeker, on 27 October 2018. The boat ran aground on rocks at slow speed. Nobody was injured, but the vessel sustained significant damage.

On August 31, 2018, the fishing vessel Master D was transiting with three crewmembers in the Gulf of Mexico 45 miles southeast of South Padre Island, Texas, when a fire in the engine room erupted. After unsuccessfully trying to extinguish the fire, the crew abandoned the vessel without injury. The fire continued to burn until the vessel sank the next day. An oil sheen of around 400 yards by 1 mile was visible in the water after the sinking. The estimated property damage was more than $162,000, the NTSB report notes.
