JTSB issued an investigation report on the collision of the container ship APL GUAM with the the container ship MARCLIFF and subsequent collision of the latter with the anchored container ship HANSA STEINBURG, off Keihin Port, in March 2019.

NTSB issued an investigation report on the contact of Cooperative Spirit tow with Hale Boggs Memorial Bridge Pier, on the Lower Mississippi River in March 2020. The report identified the pilot's ineffective actions in strong current as key cause of the incident. 

The Bahamas Maritime Authority issued a Safety Alert to share lessons to be learned from an allision with a pier structure that occurred with a pilot onboard. Preliminary investigation findings reveal the allision occurred due to a substantial deviation from the planned passage at a critical time.

Crew changes in New Zealand continued to take place over the holiday period, with 178 replacement and outgoing crew processed in December alone, Maritime NZ informed in its latest COVID-19 update.

NTSB issued an investigation report on the contact of tugboat G.M. McAllister with NGL Energy Partners berth on Elizabeth River, while engaged in manoeuvring operations. The investigation revealed excessive speed and poor communication as main causes of the incident. 

On August  5, a marine pilot fell from a pilot ladder and was fatally injured. The pilot fell while boarding a tanker inbound for the Port of New York and New Jersey.

NTSB issued an investigation report on the contact of the bulk carrier Dank Silver with a fender of the Sunshine Bridge in the Lower Mississippi River, in June 2019. The investigation identified the pilot’s decision to turn the vessel off the dock, instead of going upriver, as key cause of the accident.
