On 21 August 2017 one of the deadliest accidents in the history of US Navy took place. As the US warship 'John S McCain' was sailing in the Singapore Strait, one of the busiest waterways in the world, it collided with the Liberian-flagged chemical tanker 'Alnic MC'. The result? Ten sailors of the US warship lost their lives, while 48 more were injured, in what is considered an example of lack of effective operational oversight.

In its latest Safety Flashes, IMCA analyzed a mooring line incident which caused collision between a barge and a tugboat. Although, there was no damage as a result, IMCA stressed incorrect mooring technique and pulling angle as a key safety issue. 

The Port of Rotterdam held a major crisis exercise on the Nieuwe Maas river, near the Willemskade, where Emergency services, the Harbor Master Division and other authorities copied a scenario of a collision between a chemical tanker and a party ship.

The shipowner of the Chinese vessel that took part in the June 2019 incident near Recto Bank, has now apologized to the Filipino fishermen, whose boat was damaged. However, they highlighted that the collision was accidental. Due to the collision, Gem-Ver1 sank, and the 22 Filipino crew were in the water for several hours, before being rescued by a Vietnamese fishing boat. 

The National Transportation Safety Board issued an investigation report on the collision of two fishing vessels and the subsequent sinking of one vessel in the Gulf of Mexico in July 2018. The report identified issues related to poor lookout and inappropriate actions to avoid the collision. 

Cosco informed that its container ship CSCL Jupiter collided with another ship at Campem TCTT, in Vietnam, on 22 of August. Cosco's container ship sustained damages, but no injuries, nor environmental pollution was reported. The ship is now expected to delay around one week because of temporary repairs.

As the US Coast Guard reports, a commercial fishing vessel collided with an Italian-flagged tanker, about 20 miles southeast of Oahu, on Wednesday, August 7. The USCG further notes that there were no injuries or pollution after the incident. 

Insufficient training, inadequate bridge operating procedures and a lack of operational oversight were the key causes of the fatal collision between the USS John S McCain and the tanker Alnic MC in August 2017, the official report by the US National Transportation Safety Board revealed. 

According to the Taiwanese Coast Guard, a Taiwanese cargo vessel collided with a Chinese navy ship off Kinmen on Wednesday, causing minor damage to both ships. The Taiwanese vessel was damaged, but there were no injuries reported.

On July 24, the superyacht Blue Magic hit a pier at the port of Santa Maria di Leuca, Italy, after losing control. Before the yacht hit the pier, it first collided with a sailing yacht, which was eventually crushed between the dock and the Magic's bow. When the collision happened, the sailing boat did not have anyone on board, and no injuries were reported.
