Investigation of the accidental discharge of carbon dioxide on board a tug
The UK Marine Accident Investigation Branch (MAIB) issued the report of its investigation of the accidental discharge of carbon dioxide on board a tug resulting in serious injury to a shore-based service engineer at Her Majesty’s Naval Base Faslane on 23 August 2011.
The engineer was testing components of the tug’s fixed CO2 fire extinguishing system in the CO2 cylinder room when the accidental discharge occurred. The release occurred because pilot lines from the system’s control cabinet had not been isolated, most likely due to an incorrect plan of action.
The report recommends that coordination between vessel crew and service suppliers be enhanced.
