Investigation revealed linkage between the tiller and the rudder angle indicator
The Australian Transport Safety Bureau (ATSB) issued the report of tis investigation into the grounding of a bulk carrier at Gladstone, Queensland on 29 April 2011.
The ship grounded while departing the port. Prior to the grounding, the steering gear appeared to stop responding to bridge commands. Emergency action was insufficient to prevent the grounding. Investigation revealed that the linkage between the tiller and the rudder angle indicator had become detached because the linkage had not been installed correctly by the builder.
In addition, it was determined that the port’s safety management system had not fully identified pilotage-related risks.
