The Transportation Safety Board of Canada discusses a capsize incident, where the George H Ledcor was towing another barge and resulted to the former's capsize.The event resulted to a seriously injured crewmember and an unknown quantity of oil spill. 

TSB issued an investigation report on a possible bottom contact involving the bulk carrier 'Maccoa' in Kahnawake, in November 2018. TSB noted that when a Seaway lift bridge cannot be raised to allow the vessel to pass, the vessel’s master must take appropriate action to avoid passing the limit of approach sign.

Canada's Transportation Safety Board (TSB) issued an investigation report on the uncontrolled fall of a rescue boat during a drill onboard the RoRo passenger ferry 'Queen of Cumberland' in April 2018 off British Columbia, that caused two injuries. The investigation found that incorrect securement led to the fall. 

The Transportation Safety Board of Canada will be using a team of investigators to Vancouver Harbour, British Columbia, after a collision between the M.V. Caravos Harmony and the M.V. Pan Acacia took place on March 17. TSB did not reveal any further information regarding the damages, injuries or pollution, but it said that it will gather information and assess the occurrence.

The Transportation Safety Board of Canada released its investigation report into the collision of the dredger FRPD 309 with the loaded barge Evco 60, on 5 December 2017, in the Fraser River, after it lost its propulsion due to a blackout. The FRPD 309 sustained damage to the shell plating and forepeak tank forward of the collision bulkhead.

TSB Canada issued an investigation report on the capsizing and sinking of the fishing vessel 'Miss Cory' in the Strait of Georgia, British Columbia, in March 2017, indicating inadequate vessel stability information as the key cause. One crew member was reported missing as a result from the accident. 

In its investigation report released today, Canada's Transportation Safety Board found that inadequate vessel maintenance and voyage planning guided by an under-qualified crew led to the January 2017 grounding of the tanker 'Arca 1' near Sydney, Nova Scotia. The vessel sustained major damage to hull and propulsion.
