The US Coast Guard recently announced that a 27-year-old male crewmember fell overboard from the tanker vessel Sagami near Sabine Pass, off Gulf of Mexico.

RMI issued an investigation report on a bosun's fatal fall overboard from the general cargo ship DELICATA, off Uruguay, in March 2020. The investigation identified poor compliance with the company's SMS and safe working practices, as the crew member was not wearing a lifejacket or a safety harness when he descended to untie a pilot ladder. 

According to the French Agency Premar Mediterranee, on Monday 13 April, a crewmember onboard the chemical tanker "Elandra Baltic" fell over the sea while working on the accommodation ladder. In fact, the 55-year old Latvian has fallen during the night, from at least 6 meters high.

A German passenger from the ferry cruise ship Color Magic was found dead after falling overboard off Strømstad, in the North Sea, in the afternoon hours of Wednesday. Following notification at about 7:30 pm, Swedish Maritime Rescue Center sent a helicopter to search for a man overboard from the cruise ship. 

A crew member went overboard from the Hong Kong-flagged bulk carrier 'KSL Singapore' while the ship was 8.26 nautical miles Southwest of Orchid Island, Taiwan, according to data provided by the Philippine Coast Guard (PCG). The man was last seen onboard on 24 August.

The US Department of Defense announced Tuesday the death of a sailor who has been missing since mid July and is believed to have gone overboard from the US warship 'Abraham Lincoln', while supporting Operation Freedom's Sentinel.

In its latest Safety Digest, UK MAIB analyzed a man overboard fatality, involving the transfer of an individual from a moored RHIB to a small inflatable dinghy. MAIB stressed that the two people onboard routinely wore lifejackets when they took the RHIB to sea, but did not wear them when rowing to the mooring to bail the boat.

The UK MAIB issued an investigation report on a man overboard fatality from the fishing vessel 'Fram of Shieldaig' off Scotland, in August 2018. Alcohol was likely a contributing factor in the man's fall in the water. This was the fifth fatal person overboard accident investigated by the MAIB since October 2015.

UK MAIB issued an investigation report on the fatal man overboard incident involving the UK registered world racing yacht 'CV30' approximately 1500 nm west of Fremantle, Australia. Simon Speirs was initially secured to the yacht, but before he could be recovered his safety tether hook distorted and suddenly released. 

The Marine Accident Investigation Committee (MAIC) of Cyprus issued an investigation report on the fatality of the Master of the bulk carrier 'Evnia', after falling into the sea from a rope ladder, while the ship was berthed in Chittagong Port, Bangladesh, in June 2018.
