In Corpus Christi, Texas, two men from Matagorda Island were lifted up from the Coast Guard, in Thursday morning, after their sailing vessel ran aground because of losing its mast. A helicopter aircrew landed and helped the two men to emergency medical services experts at Port Lavaca Airport. Both the owner and the crewmember are in stable condition.

As two pilots were disembarking from a condensate tanker, the pilot ladder failed. Both pilots were unharmed, but the failure of the side rope created a dangerous situation. According to IMCA, The pilot ladder side rope had been weakened due to mould.

As IMCA informs, an electrician received an electric shock while running a new cable through an existing cable tray. Namely, his hand touched the bare end of live cable which was in the cable tray and was shocked. The wire end of the cable was not covered, as it was cut flush with cable jacket and was labelled ‘spare’.

The Transport Accident Investigation Commission issued an investigation report on the cargo hold fire onboard the multipurpose container ship 'Kokopo Chief' at Port of Tauranga, on 23 September 2017. TAIC found that the fire originated by heat radiating from an incandescent reflector lamp, as the cargo hold lights had not been switched off on completion of loading.

UK MAIB released its report about the cargo vessel 'Celtic Spirit' which collided with a survey vessel and a general cargo vessel on the River Humber, England, on 1 March 2018. UK MAIB said that insufficient anchor cable was responsible for this incident.

UK MAIB informs of a fatality on board a fishing vessel. Namely, a crew man lost his life when he fell in between two vessels. Before the fatality, the crewmembers of the fishing boat were drinking in a bar close to the harbour. Poor weather and the level of alcohol in his system is likely to have adversely affected his risk perception.

The Transportation Safety Board of Canada released its investigation report into the collision of the dredger FRPD 309 with the loaded barge Evco 60, on 5 December 2017, in the Fraser River, after it lost its propulsion due to a blackout. The FRPD 309 sustained damage to the shell plating and forepeak tank forward of the collision bulkhead.

The UK MAIB issued a safety bulletin on an enclosed space fatality onboard the fishing vessel 'Sunbeam' on 14 August 2018. The investigation highlighted lack of risk assessment and noted that the crew did not appreciate the levels of risk they were taking when entering in a refrigerated salt water tank.

According to UK MAIB, during a training passage on the south coast of England a commercially operated sailing yacht experienced a catastrophic rigging failure that led to it being dismasted. There were no injuries and the mast was cut free and released overboard.

In UK MAIB's latest Safety Digest, two crewmen broke their wrists during a sail training. Namely, the two men were turning the starboard coffeegrinder to heave in the headsail when there was a sudden change of the handles’ direction. Examination of the gearing system identified that a load-bearing pinion on the central spindle had failed.
