Lessons learned: A PFD or lifejacket is useless unless worn

In its latest Safety Digest, UK MAIB analyzed an accident concerning an open boat that was upturned in the sea. The skipper was fortunate enough to escape without using a life vessel, although he was not wearing his lifejacket, as he was carrying a waterproof mobile phone. MAIB highlighted that automatic cut-offs and other such devices should be properly maintained and tested in order to be efficient.

Fatal man overboard highlights importance of practical safety drills

UK MAIB issued an investigation report on the fatal overboard incident from the fishing vessel 'North Star', after a crewman had his leg entangled in the fishing gear and was dragged overboard. The investigation found that the vessel’s documented risk controls did not reflect the operational practice onboard, and that the crew underestimated the risks associated with a crewman becoming entangled in the back rope.

HMCG helicopter saves capsized kayakers in River Spey

Two kayakers were rescued by HM Coastguard helicopter after their kayak capsized on the 1st of November on the River Spey, near Inshriach. The Coastguard winchman was lowered down to the kayaker and placed him to a more accessible location on the other side of the river along with the female kayaker. Both kayakers were able to walk to the ambulance where they were warmed and treated for cold.

Pallet broke during lifting operations

During lifting operations part of a pallet broke causing the load to invert and drop onto the ROV hydraulic power unit equipment on deck and then onto the deck itself. Fortunately, personnel was unharmed and there was no damage to the equipment. The pallet load consisted of two drums containing lifting master links and was lifted onboard from the quay using basket slung arrangements.
