The National Transportation Safety Board has published a Marine Accident Report on the Collision between the Tows of Towing Vessels Capt. Shorty C and Jackie.

An Atlas lorry loader crane fitted on the workboat Carol Anne collapsed while being used to offload a net at a fish farm at Balure on Loch Spelve, Scotland. The crane fell onto the workboat’s skipper, who was declared deceased at the scene.

A large roll-on roll-off passenger ferry, operating on its regular route between two European ports, suffered significant damage when it made heavy contact with a linkspan as it was being manoeuvred onto its berth. There were over 500 passengers and crew on board but no one was injured and there was no pollution.

IMCA has reported an incident in which a crewman was injured during high pressure jet-washing operations. The incident occurred during wash down operations inside a ballast tank. An able seaman (AB) sustained the injury as his finger was accidently positioned in front of the water jet nozzle. The subsequent hand injury required helicopter medivac to an onshore hospital for treatment. This resulted in a LTI.

IMCA has reported an incident in which a crewman was struck by a falling object.The incident happened when a 50 Te deck crane had just recovered a high pressure running tool and was being placed back into the boom rest.

The Nautical Institute has issued Mars Report referring to a fatal accident in which several elements, each of them unremarkable, combined to create a disastrous collision and explosion at sea. Distraction on the bridge, lack of clarity about intentions, an unjustified assumption and excessive speed all played their part.

IMCA has reported an incident in which a crew person was struck by a wire under tension.The incident occurred when a rigging crew were preparing to transfer a piece of subsea infrastructure – an inline tee (ILT) - from a supply vessel to the installation vessel.

IMCA has reported an incident in which a crew member was struck by a forklift truck during quayside operations, and suffered fatal injuries. The incident happened when a vessel crew member was tasked with unloading stores from a container on the quayside. He descended the vessel gangway and headed towards the containers situated close by. It was during this short excursion that the crew member was struck by a forklift and fatally injured.

The Federal Bureau of Maritime Casualty Investigation has issued an Investigation reoport on a fatal accident on board the FV ORTEGAL TRES on May 2015 off the Shetland Islands.

Transport Malta has issued a safety investigation report occasioned by the incident of a Fatal injury to a crew member of MV Kadri at Tallinn Shipyard.
