IMCA reports serious injury during high pressure washing operations

IMCA has reported an incident in which a crewman was injured during high pressure jet-washing operations. The incident occurred during wash down operations inside a ballast tank. An able seaman (AB) sustained the injury as his finger was accidently positioned in front of the water jet nozzle. The subsequent hand injury required helicopter medivac to an onshore hospital for treatment. This resulted in a LTI.

Collision and explosion kills nine

The Nautical Institute has issued Mars Report referring to a fatal accident in which several elements, each of them unremarkable, combined to create a disastrous collision and explosion at sea. Distraction on the bridge, lack of clarity about intentions, an unjustified assumption and excessive speed all played their part.

IMCA: Crew member struck by forklift truck incident

IMCA has reported an incident in which a crew member was struck by a forklift truck during quayside operations, and suffered fatal injuries. The incident happened when a vessel crew member was tasked with unloading stores from a container on the quayside. He descended the vessel gangway and headed towards the containers situated close by. It was during this short excursion that the crew member was struck by a forklift and fatally injured.
