The Dutch Safety Board has issued report investigating a fatal accident onboard reefer ship Cool Expreso which occurred on August 7th, 2014 during transfer at sea of pallets containing frozen fish packages from the fishing vessel Annelies Ilena. At the end of the shift, the final pallet was labelled by an Annelies Ilena crew member on board Cool Expreso. The pallet shifted and trapped the crew member between the railing and the pallet, fatally wounding the crew member.

The Transportation Safety Board of Canada issued a report investigating bottom contact of roll-on roll-off passenger ferry Grace Sparkes occurred in January 2015.

ATSB has issued the report of its investigation of the breakaway of Grand Pioneer and AAL Fremantle in Fremantle on 17 August 2014. The two ships broke away from their berths when a thunderstorm passed across the Port of Fremantle. A bollard on the wharf holding both ships’ stern lines failed, most likely after Grand Pioneer’s vehicle ramp contacted it. AAL Fremantle contacted a ship at an adjacent berth, and parts of the Fremantle Rail Bridge nearby.

ATSB has issued report regarding a fire which started in the engine room of the bulk carrier Marigold, while it was loading a cargo of iron ore in Port Hedland, Western Australia. Firefighting by the ship’s crew included activating the Halon gas fixed fire suppression system for the engine room. However, a full release of Halon gas did not occur, nor was the engine room effectively sealed. Consequently, the fire continued for about 12 hours until it burnt itself out.
