With respect to the failure of the rescue boat lift, CHIRP advises that:
- Cross-checking, supervision and testing are always essential.
- It is a good practice to take an equipment replacement and draw accompanying notes.
- LSA and service manuals should also be consulted and kept up to date.
- But the most step to avoid this tragedy is a simple one: Better design.
