The Australian Maritime Safety Authority issued its monthly review of casualties in Australian waters, informing of a total of 42 domestic commercial vessel incidents in the month of November. From these, three were serious
- A passenger vessel experienced a control failure, resulting in a collision with another vessel that caused damage to both vessels at NSW.
- A hire and drive barbeque boat with passengers on board had a fire ignite from the cooking device. The fire was unable to be extinguished by passengers. Yet, no injuries were noted passengers at VIC.
- The person in charge of the vessel suffered a heart attack and the patient was taken to hospital by paramedics. Unfortunately they passed away in hospital at NSW.
Submit incident report: Within 72 hours after becoming aware of the incident, submit Form 19— Incident report. The incident report provides detailed information about the incident, in particular the measures put in place to prevent reoccurrence.
