In the latest issue of its Safety Digest, UK MAIB presents a collision of a tug with its towing vessel, due to wrong judgement and poor communication. The report provides a description of what happened and important lessons learned to prevent similar accidents in the future.

USCG official investigation report on El Faro cargo vessel found that the captain underestimated the strength of a hurricane and overestimated the ship's strength on a trip between Florida and Puerto Rico. Also, ship's owner, Tote Maritime Inc., had not replaced a safety officer and had violated regulations regarding crew rest periods and working hours.

As the fishing vessel Venessa S was approaching Cabbage Tree Island, on 20 June 2017, the engine stopped and propulsion was lost. The skipper restarted the engine after several attempts, however it was too late to avoid striking the bank. Venessa S struck the bank and started taking on water.

On Friday noon, 68 passengers and crew members onboard the Robben Island passenger ferry "Thandi" were evacuated, after the ship was reported to be taking water, off Table Bay, South Africa, the Transnet National Ports Authority informed.

Twenty crew members of a Tanzanian-flag ship were rescued by an Omani Good Samaritan, after the vessel sank on Saturday, 3 September, in Omani waters, according to the Royal Oman Police. The ship, that was en route from Sharjah to Eritrea, sank just before the port of Al-Labki in Wilayat Jazir.
