NTSB released a report about the the vehicle carrier Alliance St. Louis. The vessel had began from Port Arthur, Texas, heading to Jacksonville, Florida, when a pipe plug on the fuel pump for the main engine’s no. 6 cylinder came loose, causing fuel spray onto the engine’s hot exhaust gas pipe manifold. The atomized fuel ignited, causing fire.

On March 24th of 1989, one of the most catastrophic – if not the most catastrophic – oil spills of the 20th century took place. The tanker Exxon Valdez was departing the Port of Valdez, Alaska with a full load of North Slope crude oil.

On 24 February 2017, 'Honor' was underway in the English Channel, when a fire broke out in the upper vehicle deck. The fire was extinguished by the crew using the vessel’s CO2 fixed firefighting system. The accident resulted in one injury and extensive damage, of more than $700,000, to the Honor’s hold as well its cargo.

On 6 September 2016, at 0005, the tanker 'Aframax River' allided with two mooring dolphins in the Houston Ship Channel, near Deer Park, Texas. The allision punctured the ship’s hull plating, and about 88,000 gallons of low-sulfur marine gas oil spilled into the water. The oil ignited and burned for about 45 minutes.

On June 9, 2017, the towing vessel 'Marguerite L. Terral' was pushing a flotilla of six cargo barges downbound on the Atchafalaya River, Louisiana, west of Baton Rouge. At 1428 local time, the starboard lead barge in the tow 'RM 3367' and the barge immediately aft 'RM 3304B' contacted a pier of the Union Pacific Railroad Bridge at mm 41.5. The allision resulted in damage to both barges and the bridge, totaling more than $4 million in repairs.

NTSB released Wednesday its full report on the sinking of the 'El Faro' cargo ship in October 2015, the deadliest shipping disaster involving a US-flagged vessel in more than 30 years, confirming that the tragedy was attributable to the captain’s insufficient action.

On 6 March 2017, 'St. Dominick' grounded in Pumicestone Bay, Alaska. The engine room flooded within 10–20 minutes of the grounding, and the four crew abandoned the vessel a short time later. The vessel, valued at $1.1 million, was deemed a constructive total loss.

In November 2016, the fully-laden bulk carrier 'Nenita' was outbound on the Columbia River, when it suffered an engine failure impacting its ability to maneuver. The vessel subsequently ran aground at Three Tree Point on the Washington State side of the river, damaging its bulbous bow and hull.

The US NTSB issued an investigation report on the sinking of the motor vessel 'Exito' in the Bering Sea, Alaska, in December 2016, that caused two fatalities, a significant environmental damage, and a total ship loss. The probable cause of the sinking was progressive flooding from an undetermined location.

On 3 June 2016, the Malta-flag cruise ship 'Celebrity Infinity' allided with berth 3 in Ketchikan, Alaska, sustaining a 9-inch-diameter hole on the forward port side. The berth suffered extensive damage to the catwalks and pilings of an estimated cost about $1.15 million. The NTSB issued an investigation report on the accident.
