Maritime NZ issued its Lookout report, analyzing a series of maritime casualties and providing useful lessons learned. In the first of the cases, the agency describes a fire in the cargo hold of a container ship due to the heat from a 500 watt light bulb, while the ship was in port on NZ East Coast in late 2017. 

On the aftermath of a keel failure which caused the UK-registered commercial yacht 'Tyger of London' to capsize off Tenerife, Canary Islands, back in December 2017, the UK MAIB issued a safety bulletin for owners of yachts fitted with shallow draught, lead keels.

In its Monthly Safety Scenario for August, the Swedish P&I Club presents a case of ships collision due to restricted visibility as a result from fog. The incident highlighted safety issues related to compliance with COLREGs, on lookout, safe speed and conditions on restricted visibility.

The ship was standing by at location due to fog preventing operations. At 03:20 the high-level alarm on the forward bilge sounded. On investigation of the alarm, the forward bilge was found to be full of fuel oil. The fuel was seen to be spraying from the port main engine onto the engine room plates.

As IMCA notes, a saturation diver suffered a lost time injury to his left arm during subsea high-pressure water jetting operations. The incident took place when the water jet gun failed, and a part came off the gun. A mixture of causes led to this accident. Equipment failure, improper training and maintenance all played a significant role.

IMCA informs about an incident where a crewman received a serious electric shock when his hand touched two loose wires hanging from the bottom cable entry pop-out of a distribution board. The incident happened while he was assessing cable routing from the distribution board during a mobilisation.

The US House of Representatives approved the legislation to address maritime transportation safety issues raised by the El Faro sinking, promote the US Coast Guard’s awareness of technologies that could help improve Service mission performance, and reduce marine debris.

TAIC issued a report on the grounding of the cruise ship 'L’Austral', at Milford Sound, on 9 February 2017, revealing an insufficient use of electronic navigation systems. The ship was involved in another incident one month earlier, when it entered an unauthorised zone and struck an uncharted rock.

Transport Malta's MSIU issued an investigation report on the grounding of the Maltese registered container ship 'Kea Trader' in the Pacific ocean. In a first of its kind situation, the six-months-old ship, owned by Lomar Shipping, ran aground on 12 July 2017 and remains on site until today.

On July 23 1998, a destructive fire took place on board the car carrier "Eurasian Dream". The fire spread quickly, and despite the efforts from the crewmembers and fire-fighters, the vessel and a large amount of its cargo became a total loss.
