Two crew members were killed and another two were injured during cleaning a piping system onboard the Marshall Islands-flagged bulk carrier 'AP Dubrava' in the afternoon hours of Wednesday. At the time of the incident, the ship was sailing about 150 nm offshore Brazil.

A news study by the US NAVSAFECEN showed that the boots used with the Type III Navy Working Uniform are not related to an increase in ladderwell injuries aboard ships, as it was previously reported. One of the most common mishaps reported to the Naval Safety Center is slips, trips, and falls on ladders onboard.

The US Coast Guard responded to a boat collision that resulted to four injured passengers aboard two vessels on Sunset Lake near Wildwood, Monday, July 15. The injured passengers were transmitted to Schooner Island Marina where local EMS were standing by to provide assistance. 

In the latest Safety Digest, UK MAIB focused on a serious back injury caused when a RHIB tried to cross the waves of a passing vessel. The incident took place off the South coast of England, while eight passengers enjoyed a teambuilding exercise. UK MAIB highlighted that it is of a great importance to always call and inform the Coast Guard when an injury takes place on board, as they will be able to handle a critical situation.

US Coast Guard Marine Safety Unit Lake Charles is carrying out a Marine Casualty Investigation in cooperation with the Louisiana Department of Wildlife and Fisheries Law Enforcement Division to determine the cause of June 3rd vessel allision with a dredge pipeline operating on the Calcasieu River Ship Channel near Hackberry, La. The incident led to one loss of life and one injury.

Just before 8.30pm Tuesday, HM Coastguard received a VHF radio broadcast from a fishing vessel with six people onboard 70nm north east of Stornoway, reporting that one of their crewmen had sustained a hand injury. The Stornoway coastguard helicopter flew to the scene and winched the fisherman.

An explosion was reported on board the LPG tanker Syn Zania on July 1st. After the explosion a fire erupted, while one died and 15 crew members were injured and evacuated to receive medical treatment. The ship was moored at a port in Turkey.

The US Coast Guard released Findings of Concern on 'High Speed Craft Operations Manuals,' regarding an incident where a high speed catamaran passenger ferry allided with a rock jetty at over 30 knots, injuring 15 passengers.

An explosion took place onboard a tanker’s engine room at the Port of Makhachkala, Russia, on June 11. The explosion occurred while oil products were being pumped at the port, while three people died, and three others were transferred to the hospital.

A crane collapsed at Yangzijiang Shipbuilding’s Yangzi Xinfu yard, on June 10, falling into a bulk carrier which was under construction at the time. The accident caused extensive damages to the ship, while two workers were injured, one of whom died. Yangzijiang Shipbuilding provided a comment into the incident, saying that the crane collapsed due to overweight lifting operations.
