In its latest Safety Digest for 2019, UK MAIB describes a case of a barge grounding due to failure of towing wire. With respect to this incident, UK MAIB advised that companies should therefore include a time frame for replacement of the towing wire eye hard socket within their operating procedures. 

Complying with the ISM Code is at least a prerequisite for a safe navigation. As part of its series on ISM Code-related accidents, SAFETY4SEA focuses on the grounding of the Maltese-registered tanker ‘Ovit’ in the Dover Strait, off UK, in September 2013. 

The passenger vessel Galápagos Majestic ran aground at the north coast of Santiago island in the Galapagos Archipelago, Ecuador, during the morning of May 31st, with 26 people on board. The vessel's hull was breached in the bow area and started to list to starboard side.

According to the USCG, the Guard along with the Capital City Fire/Rescue saved six people when their boat ran aground near Juneau, on Saturday, May 1. After saving the people, the Coast Guard crew anchored the grounded vessel and the owner plans to go and retrieve it during high tide.

“The scene at Nightingale is dreadful,” authorities were quoted as saying after the Maltese-registered bulk carrier ‘Oliva’ ran aground in the South Atlantic Ocean, causing an unprecedented oil spill in one of the most pristine regions in the world.

DMAIB has issued an investigation report on the sinking of the small German motorboat RIGI, in Faxe Bay, Denmark, about 1 nm from shore. The investigation showed that the choice of boat, its equipment, its operation, and its manning influenced the events on the day of the accident. 

In 2007 the bulk carrier ‘Pasha Bulker’ came out at the Nobbys Beach of New South Wales offering an unusual spectacle for local people. The incident is an interesting case study of inadequate communication, inefficient SMS, poor judgement due to fatigue and the objective cause of extreme weather conditions.

Japan Transport Safety Board informed that it launched 922 investigations in 2017. The majority of these cases included collisions, while 1,199 vessels were involved in the accidents, most of which were fishing vessels and pleasure boats. Of the 922 investigations launched in 2017, 216 were cases of collision, 182 cases of grounding; 138 cases of fatality/injury; and 104 cases of contact.

The German Bureau of Maritime Casualty Investigation published its report on the Panama-registered bulk carrier Glory Amsterdam, which ran aground about 1.6 nm north of the German North Sea island of Langeoog. The BSU reported that the ship's command of the Glory Amsterdam had not unequivocally understood the Nordic's mission and legal basis upon which she was operating at the scene

The bulk carrier Solomon Trader is possible to be refloated and towed from the reef at Lavagu Bay, West Rennell, Solomon Islands. The operation could take place within a couple of weeks. The Hong Kong-flagged bulk carrier 'Solomon Trader' was loading bauxite when it ran aground due to unexpected gale at Kangava Bay, Rennell Island, on 4 February, causing an environmental disaster.
