Deadly Maritime Accident in Antarctica Kills Three Crewmembers

Fire broke out on board the vessel At least three crewmembers on a fishing vessel in the Antarctic are believed dead after a fire raged on board the vessel. At least 37 crewmembers have been rescued from the fire.The South Korean fishing vessel, the Jung Woo 2 ran into trouble in the Ross Sea, which is about 370 miles north of the US McMurdo Station Antarctic Base. The fire originated in the living quarters of the vessel. It quickly spread to the engine room and fish processing plant. By this time, the flames were completely out of control and the ship's limited firefighting equipment was insufficient to put out the flames.Crew members sent out a distress call, and rescue personnel from New Zealand reached the scene. Some of the men were able to make their way to a life raft. The vessel had two rafts, but the other raft was burned, and the remaining men had to wait on the burning ship until help arrived. Waste oil incinerator catches fire

Experience Feedback During routine watch keeping, the engineer in charge started the waste oil incinerator for burning garbage and waste oil sludge. After about an hour of operation, the ship's fire alarm sounded and the local fire (hyper mist) extinguishing unit was activated in the waste oil incinerator space. On hearing the alarm, all personnel mustered and the incinerator was stopped.Result of investigation1. The atomiser unit's air nozzle holes were found to be choked with hard viscous sludge thereby restricting the flow of air into the incineration chamber. This condition seemed to have existed for some weeks prior to the incident;2. Waste oil had failed to atomise properly and had collected and spread over the bottom of the combustion chamber and ignited, producing a large quantity of smoke;3. The smoke activated the fire alarm, triggering the local fixed water mist fire extinguishing system.Root cause/contributory factors1. Failure to maintain the incinerator's burner assembly as per maker's recommendations; in particular the atomiser nozzles had not been properly inspected and cleaned;2. At Mumbai harbour The Times of India reports that a fire broke out on Saturday, March 24, on board of Korean 9500 DWT chemical tanker 'Royal Diamond 7′ at Mumbai harbour injuring seven people."At 11.25 am, the Korean owned 9500 DWT chemical tanker 'Royal Diamond 7′ reported to the Mumbai port's vessel traffic system/control station that an explosion had occurred and three crew members have been injured," Mumbai Port Trust (MPT) press statement said.Upon receipt of information about the fire, it was ensured that the victims were brought to shore and provided with necessary medical assistance. Fire out in Nigeria, Chevron says

The well stopped flowing on its own A natural gas well off the coast of Nigeria stopped flowing by itself after a fire burned out at the site, the Nigerian division of Chevron announced."Chevron Nigeria Ltd. announced that the fire at the site of the Funiwa 1A natural gas well offshore Nigeria ceased burning on Friday," the company stated. "The well stopped flowing on its own."The company said no natural gas had flowed since the fire went out but monitoring was ongoing.More than 150 rig workers were pulled from the K.S. Endeavor rig off the Nigerian coast in January after the rig partially collapsed. Chevron initially said it observed a small sheen from the area, which represented about 13 barrels of liquid.The drilling of a relief well to extinguish the fire began in January. CNL said it hired local residents to monitor areas beaches for possible contamination. It added that it offered food and other supplies to communities "to recognize the help and support that they have given the company."Two rig workers were declared dead following a search-and-rescue operation.The cause of the accident remains under investigation.Source: UPI

Endeavor, a drilling rig offshore Nigeria operated by FODE Drilling Nigeria Limited. While Chevron's highest priority remains the rescue of the missing contractors, the company continues to devote the necessary resources to resolve the rig incident in a responsible and timely manner.As previously reported, the K.S. Endeavor was drilling a natural gas exploration well for Chevron Nigeria Limited (CNL).
