The Office of Merchant Mariner Credentialing provided guidance on the affidavits required with an application for a duplicate merchant mariner credential (MMC) and mariner medical certificate.
In accordance with Title 46 Code of Federal Regulations 10.229, mariners who have lost their MMC or medical certificate may be issued a duplicate.
Mariners applying for a duplicate credential must submit an affidavit describing the circumstances of the loss.
Once an affidavit is submitted as part of an application it is evidence that the applicant is affirming that the information provided is true and correct.
The affidavit may be submitted in writing or electronically. The USCG may verify the affidavit as part of the evaluation of the application for a duplicate credential.
