CHIRP released its latest video, in which it focuses on reports about dangerous practices when working aloft, and others referring to leisure vessels and ship design, as well as problems faced by pilots during their duties.
Namely, in the video, CHIRP presents three cases.

In the first case, it describes an encounter between a yacht and a large passenger vessel, where navigation lights were difficult to distinguish amongst other deck lights.
The second case a poorly designed bridge wing platform, caused a serious hazard for a pilot, which had the potential for a fatal fall from height.
For the last case CHIRP presented the case of yacht which had suffered engine problems while on passage and the skipper requested a tow from the mouth of the river to his berth at the yacht club. A considerable swell in the entrance of the river resulted in a rigid inflatable boat (RIB) and the yacht to be pulled apart and then back together with quite some force.
