UK MAIB issues its Safety Digest 1/2016

The UK MAIB has announced the publication of the first issue for 2016 of its Safety Digest which includes lessons learned from maritime accidents. This latest edition of the Safety Digest contains 25 articles which are examples of poor risk awareness demonstrated by the crews of vessels. The publication once again highlights that a cautionary approach should be second nature to every mariner about to start a task or embark on a course of action on the bridge, in the engine room or on deck.

UK MAIB report on Hoegh Osaka accident

The UK MAIB issued report on the investigation into the listing, flooding and grounding of Hoegh Osaka in Bramble Bank, The Solent, UK on 3 January 2015. Stability modelling and analysis following the accident show that Hoegh Osaka heeled heavily to starboard while turning as a result of having departed port with inadequate stability.A key finding of the MAIB investigation is that no departure stability calculation had been carried out on completion of cargo operations and before Hoegh Osaka sailed.

Mooring line failure results in serious injury onboard LNG carrier

On 2 March 2015, a deck officer on board the LNG carrier, Zarga, suffered severe head injuries when he was struck by a mooring rope that had parted while repositioning the vessel at the South Hook LNG terminal, Milford Haven. The officer, who was in charge of the vessel’s forward mooring party, was airlifted to a specialist head injuries trauma unit for emergency surgery

MAIB report on collapse of mezzanine deck on board ferry

On 18 July 2014, the starboard forward mezzanine deck on board the UK registered roll-on roll-off passenger ferry St Helen partially collapsed, causing injuries to a crewman and several passengers. UK MAIB has issued investigation report which reveals that the failed wire rope had suffered a significant amount of mechanical wear. The mechanical wear, which had primarily been attributed to internal and external abrasion caused by a lack of lubrication, had severely diminished the strength of the rope.

UK MAIB issues report on container vessel and tanker collision off UAE

The UK MAIB has issued a report on the investigation of the collision between the container ship Ever Smart and the oil tanker Alexandra 1 on February 11,2015 near the entrance to the buoyed approach channel in Jebel Ali, United Arab Emirates. The container ship was outbound at a speed of 12 knots and had disembarked its pilot. The tanker was inbound and was moving very slowly ahead while waiting for the pilot from the container ship to board. Both vessels suffered major structural damage to their bows but there were no injuries or pollution.
