Norwegian police named three suspects over the collision of the Norwegian frigate 'KNM Helge Ingstad' with the the Maltese-flagged oil tanker 'Sola TS' off Bergen, in November 2018. The warship sustained significant damage from the collision and sank a few days later.

The US Coast Guard terminated the voyage of the 45-foot pleasure craft 'Sea You Twerk' with 14 passengers aboard Sunday in Biscayne Bay, Miami, after discovering multiple safety violations. With this respect, USCG reiterated potential fines for illegal charter vessel operations. 

Norway's Petroleum Safety Authority completed the investigation of the movement-compensated walkway incident on the Tambar field in July 2018, revealing several breaches of safety regulations. Following the Mumbai terror attacks in 2011, the Directorate General of Shipping (DGS) in India banned the use of “Thuraya, Iridium and other such satellite phones” in Indian waters. However, crew members continue to be penalized by Indian authorities for violating the ban, according to data provided by Gard P&I Club.

The Petroleum Safety Authority Norway has completed its investigation of an incident involving a dropped object on the Jotun B facility on 19 May 2018. The authority informed that it found a number of breaches of the regulations. PSA asked Point Resources, Jotun B's operator, to explain by 15 October how it will deal with the non-conformities.

The US Coast Guard announced that it terminated the voyage of a vessel violating small passenger laws and regulations in Lake of the Ozarks, Missouri, on Saturday. This constitutes the eighth illegal passenger vessel voyage termination on Lake of the Ozark since 4 June 2018.
