In its latest Safety Digest, UK MAIB describes an incident where a yacht collided with a whale. Initially the crew did not notice any damages on the ship, while a slight ingress of water was being managed by the bilge pumps. However, as the time was passing more problems were beginning to arise, leading the crew to abandon the boat.

UK MAIB presents an accident in its safety digest where a foreman fell through a hatch and down two decks to the bottom deck. He was later taken to hospital to be treated as he sustained multiple fractures. It is not known what caused the foreman to stumble and fall, but when working at height or when there is a risk of falling, vigilance is very important.

At its latest safety digest, UK MAIB describes an incident involving a laden bulk carrier and a tug. Namely, as the bulk carrier was approaching the lock entrance, and when the tug put weight on the line, it parted. The bulk carrier contacted a pier, sustaining damages to its port side.

When about a mile offshore, the skipper noticed the vessel take a slight list to port and moved aft from the small wheelhouse to investigate. He saw steam coming out of the engine compartment and the deck quickly flooding with water. Before he could reach the VHF, the boat capsized to port and then sank by the stern.

In its latest Safety Digest, UK MAIB describes how a tug girted and then capsized, while assisting a UK-flagged container ship departing from port. The accident, mostly attributed to inexperience and lack of coordination among the bridge team, pilot and tug crew, resulted in two fatalities.

While a coastal Ro-Ro passenger vessel was at berth, it was using a auxiliary boiler to provide onboard accommodation heating, while the engines were not operating. After the boiler began working, fire broke out near the boiler, caused by a leak. The engineers quickly shut the boiler down and put the fire out with a foam fire extinguisher.

In its latest Safety Digest, UK MAIB analyzed a collision between two rigid-hulled inflatable boats, after a windsurfing race, which caused four people to fall overboard. According to MAIB, overconfidence on the boats' handling was critical to the accident, while the fact that everyone was wearing life-saving equipment was vital to prevent loss of life.

The UK MAIB presented a submarine's hit into a fishing vessel's gear, which put the fishermen's life at great risk. UK MAIB emphasized that the responsibility for collision avoidance between a dived submarine and any other vessel rests entirely with the submarine’s command team. In this case, the crew misinterpreted the noise of the fishing vessels as a noise coming from a merchant ship.

In the latest edition of its Safety Digest, the UK MAIB described a collision of a cargo ship with a bunker barge, highlighting the possible consequences from inefficient watchkeeping, as well as poor understanding of a vessel's stability. Keeping a good lookout is perhaps the most fundamental watchkeeping requirement on any vessel.

A tanker and an anchored fishing vessel came into brief contact, after the pilot of the tanker realised the two vessels were too close to avoid contact and the crew of the fishing vessel had not realise the tanker was close.
