The NTSB issued an investigation report on the grounding and sinking of the fishing vessel 'Southern Bell' in the Gulf of Mexico, east of Sabine Pass Jetty Channel, in October 2017, that resulted in a release of approximately 3,800 gallons of diesel and oil into the water and in vessel total loss.

The Transportation Safety Board of Canada issued an investigation report on the grounding of articulated tug-barge composed of the tug' Nathan E. Stewart' and the tank barge 'DBL 55', resulting in the barge's sinking and an oil discharge into the water. The report indicated fatigue as the key cause of the accident. 

DNV GL released a new report providing recommendations on how to perform wellhead fatigue analysis, and describes the main challenges related to wellhead fatigue analysis in the oil and gas industry. Almost 50% of senior oil and gas professionals said that there is no significant investment in inspection and maintenance of infrastructure and equipment.

UK MAIB released its report about the accident that took place on 1 July 2017, when two Hong Kong registered vessels, the bulk carrier Huayang Endeavour and the oil tanker Seafrontier, collided in the Dover Strait about 5 nautical miles west of Sandettie Bank. Both vessels were damaged, but were able to proceed to nearby ports.

Last year, the findings of a key industry report by 'Project MARTHA' were presented, identifying growing levels of fatigue, particularly among Masters and Watch Keepers and noting that motivation was a major factor in fatigue experienced by seafarers. 

On 6 March 2017, 'St. Dominick' grounded in Pumicestone Bay, Alaska. The engine room flooded within 10–20 minutes of the grounding, and the four crew abandoned the vessel a short time later. The vessel, valued at $1.1 million, was deemed a constructive total loss.

In October 2016, the articulated tug and barge Nathan E Stewart/DBL 55 ran aground on Edge Reef off Athlone Island, Canada, causing significant environmental damage, as 29,000 gallons of oil were released. Damage to the vessel and barge was estimated at $12 million.

In April 2017, while transiting northbound through the Panama Canal, the Panama-flagged tugboat 'Cerro Santiago' collided with the USCG cutter 'Tampa' in Miraflores Lake, Panama. Although the tugboat was not damaged, the cutter sustained $170,018 in damage to the stern, as well as to various systems in the steering gear room.

AMSA issued a marine notice to draw the attention of vessel owners, operators and seafarers to the need to comply with the fitness for duty requirements, including hours of rest, under the STCW Convention and the MLC Convention.
