UK MAIB 22/2013
UK MAIB has issued accident investigation report on MV Amber which grounded on thesouth shore of the River Thames shortly after departingfrom Tilbury power station on November 15, 2012.
The vessel’s bridge teamlost situational awareness in dense fog as the vesselmanoeuvred from the berth on the north shore, beforegrounding on the opposite side of the river.
The MAIB investigation found that the accident was caused by the bridge team’sloss of situational awareness as the vessel left the berth in restricted visibility.Therolesandresponsibilitiesofthebridgeteamhadnotbeenconfirmedbeforedeparture, no continuous radar watch was kept and the vessel’s position, course andspeed were not effectively monitored during the manoeuvre.
