Safety Bulletin in response to a tragic accident The Marine Accident Investigation Branch (MAIB) has published a Safety Bulletin in response to a tragic accident which happened on the car carrier Tombarra, whilst the vessel was berthed at Royal Portbury Docks, Bristol, UK on 7 February 2011.Investigations have revealed that a malfunction of a proximity switch resulted in failure of a fall wire. The accident occurred as the rescue boat reached its stowed position on the davit following a monthly drill. Report on the investigation of a fire and explosion on board a bulk carrier

Hotwork not performed in accordance with established safety procedures The UK Marine Accident Investigation Branch (MAIB) issued the report of its investigation of the fire and explosion on board a self-unloading (SUL) bulk carrier in western Scotland on 2 July 2010.The fire probably was the result of hotwork recently performed near the cargo conveyor belt. The hotwork was not performed in accordance with established safety procedures. Elements of the conveyor belt are highly flammable.There were no effective means of early detection or containment of the fire, which spread to the engineroom and nearby spaces used for chemical storage. Seafarer dies during lifeboat drill

Seafarer died follwing heart attack A Filipino seafarer has died and three other Filipino crewmembers were injured on 7 February during a lifeboat drill on a car carrier, the Norwegian-owned, UK-flagged Tombarra, berthed at Bristol, England.The seafarer died following a heart attack after the boat fell about 30 metres into the water. The incident follows recent warnings on lifeboat risks (see news story 28 January 2011).The UK Maritime Accident Investigation Branch has suggested that 16 per cent of all lives lost on merchant ships have been in lifeboat accidents.Source: ITF

Vessel collision leaves two fishermen in need of rescue

Both have been recovered A collision between two vessels 30 miles south of Start Point in Devon left two fishermen in the water, both of whom have now been recovered.The collision, between a container ship the Boxford and a fishing vessel, the Admiral Blake, happened at 18.35. At 18.43, Brixham Coastguard received a mayday call that had been relayed by another fishing vessel (the Amber J) on behalf of the Admiral Blake, reporting that they had been involved in a collision and that they had a man overboard. One other crewman had also fallen overboard and although he had been recovered back on board he was cold and wet. There were two other people on board who were reported to be safe and well. They also stated that they were not in danger of sinking, however they were taking on water. Brixham Coastguard launched a major search for the man overboard, sending the Coastguard Rescue helicopter from Portland to the scene, along with launching the Salcombe Lifeboat. Report regarding an accident on board a Delta 8.5m RIB on River Thames

One passenger injured The UK Marine Accident Investigation Branch (MAIB) released the report of its investigation regarding an injury occured to a passenger on board a Delta 8.5m RIB on the River Thames, London on 6 May 2010. A male passenger on a Delta 8.5m rigid inflatable boat (RIB) suffered lower back compression fractures while the boat was transporting him, with fellow workers, to a jack-up rig on the River Thames. The injury occurred as the passenger landed heavily on a locker lid, where he had been sitting, after he had been momentarily lifted off the lid due to the motion of the craft.You may view the report here.Source: UK Marine Accident Investigation Branch
