MAIB issues report re vessel CMA CGM Platon

There was no pollution and no one was hurt The MAIB issues Report 26/2011 regarding vessel CMA CGM Platon. On 15 May 2011, the UK registered container vessel CMA CGM Platon made heavy contact with a quay on the south bank of the River Thames. The quay sustained superficial damage but the vessel suffered significant damage to her bow, and her forepeak tank was punctured.Fortunately there was no pollution and no one was hurt.CMA CGM Platon had sailed from a berth at the Northfleet Hope Container Terminal at Tilbury at 0958. The pilot had conducted the manoeuvre, under similar tidal conditions, on a number of occasions without incident.The MAIB investigation found that the pilot had applied port helm too late to prevent the vessel's bow from swinging to starboard once it entered the tidal stream.For more information, click here.Source: MAIB

Portsmouth-bound ferry passenger’s 20-hour wait due to escape fire criticised

MAIB says Portsmouth was delayed through ineffective co-ordination Poor on-shore co-ordination and defects in on-board equipment meant passengers had to wait up to 20 hours at Portsmouth Harbour before getting off a ferry after a fire broke out, an accident report said today.The roll-on, roll-off ferry Commodore Clipper was close to the harbour when the fire started on the main vehicle deck on a crossing from Jersey.But berthing at Portsmouth was 'significantly delayed through ineffective co-ordination between shore agencies and because of equipment defects', the report by the Marine Accident Investigation Branch said.The report went on: 'Once alongside, the high density of cargo and constraints in the design of the vessel limited access to both fight the fire and to disembark the passengers.'As a consequence, freight trailers had to be towed off the vessel before the fire could be extinguished. The last of the 62 passengers disembarked from the vessel nearly 20 hours after the fire started.'The incident, on the 14,000-tonne Bahamian-registered ferry, happened at 2.42am on June 16 last year. No-one was injured.The MAIB said that no single person or organisation had responsibility for the whole incident. UK MAIB issues report on collision in the Irish Sea in April 2011

The "Philipp" has "failed to stop" after a collision with the "Lynn Marie" The "Philipp" has "failed to stop" after a collision with the scallop dredger "Lynn Marie" six miles off the Manx coast in the Irish Sea in April 2011, according to an official report released now.The Marine Accident Investigation Branch report recommended raising crew competency onboard the "Philipp". It also advised the container ship managers to ensure navigation and anti-collision aids were used properly. After the collision the container feeder did not stop and no attempts were made to communicate with the fishing vessel to see if it required assistance.The report concluded that the watch keeper onboard the "Lynn Marie" did not notice the container ship's direction change towards his vessel until it was too late.You can view the MAIB report by clicking here.Source: MAIB

UK MAIB issues report on fatal accident of rescue boat

Malfunction of a proximity switch, which resulted in failure of a fall wire At approximately 1550 (UTC) on 7 February 2011, the fall wire of the rescue boat on board the UK registered car carrier Tombarra parted when the vessel was alongside in Royal Portbury Docks, Bristol, UK.The accident occurred as the rescue boat reached its stowed position on the davit following a monthly drill. Hoisting was not stopped before the davit reached its stowed position.The proximity switch, that should have cut electrical power to the winch motor before the davit reached its stops, failed to function.The rescue boat and its four crew fell nearly 29m into the water below. Master and other deck officers gathered on the bridge to celebrate an officer's birthday The UK Marine Accident Investigation Branch (MAIB) issued the report of its investigation of the grounding of a feeder container ship on the south coast of Spain on 15 February 2011. The container ship was en route from Algeciras to Valencia. Shortly after midnight on 15 February, the master and other deck officers gathered on the bridge to celebrate an officer's birthday. A number of alcoholic toasts were consumed.The celebration ended at about 0200, leaving the officer of the watch alone on the bridge. The ship/s voyage data recorder reveals that shortly thereafter the ship's course was radically changed. The chief mate, who normally stood the 0400-0800 watch, arrived on the bridge at about 0600 to find it unmanned and the ship aground with the engine at full speed. The master was called and came to the bridge minutes later.At about that time, the Spanish Coast Guard called to inquire as to the status of the ship.
