Transport Malta investigation: Ship’s grounding linked to bank effect

Transport Malta MSIU issued an investigation report on the grounding of the Maltese-registered container ship ‘APL Danube’ in April 2019. The investigation revealed that the route on the ECDIS had been deactivated, while the Captain was away from the console during ship’s manoeuvre.

The Swedish Club’s Navigational Claims issue highlights the importance of double-checking the vessel’s passage plan to ensure that the vessel’s navigational path is correct according to all data entered into the navigational equipment. 

Australian cutter grounding linked to poor use of ECDIS

Australia’s Transport Safety Bureau (ATSB) issued an investigation report on the grounding of the Australian Border Force cutter Roebuck Bay (ABFC Roebuck Bay) on Henry Reef, Queensland, in September 2017. The report identified an amendment on the passage plan which led the vessel to the reef and poor use of ECDIS.

Ships collision in Marmara Sea linked to poor lookout

Transport Malta’s MSIU issued an investigation report on the collision between the Ro-Ro cargo vessel ‘Neptune Hellas’ and the general cargo vessel ‘Nur’ while underway in the Marmara Sea. The investigation identified issues related to poor lookout, as well as ineffective use of electronic navigation and radar information. 

Officer forgets to insert a waypoint in GPS, ship runs aground

In its Monthly Safety Scenario for November, the Swedish P&I club analyzes a case of a ship grounding due to insufficient checking of the passage plan. When creating the passage plan, it is suggested that the plan is double checked by another officer to ensure all waypoints have been selected.
