The injured man was given first aid on board the ship and at 1830 on 11 October, when it anchored off Eden, New South Wales, he was evacuated and hospitalised.On 12 October, when the ATSB was notified of the incident, it initiated an investigation. Two ATSB investigators travelled to Eden on 13 October to conduct an on-site investigation, interview involved parties and collect relevant information. The evidence obtained will be analysed and a draft investigation report compiled. When the draft investigation report is approved for release, it will be forwarded to directly involved parties for comment.A report has not yet been released for this ...

Investigation report issued by ATSB The Australian Transport Safety Bureau (ATSB) issued recently the report of its investigation of a crew member fatality following a fall on board a bulk carrier at sea on 2 February 2011.Investigation revealed that the bosun fell into a cargo hold when he lost his balance while standing on the hatch coaming. He was attempting to free a cargo runner that had jammed in the head of a davit the crew was using to lift iron ore residue from the hold.The davit had been in use for several years and jammed not infrequently. Normally, the davit would be swung back over the deck before clearing the jam. It is not known why the bosun elected to climb onto the hatch coaming on this occasion.No formal safety assessment regarding use of the davit on this ship had been done.To view the report, click here.Source: ATSB

Died after falling from the ships number five cargo hold ladder The Australian Transport Safety Bureau (ATSB) issued the report of its investigation into a crew member fatality on board a bulk carrier at sea off Queensland on 15 November 2010.The individual died after falling from the ship's number five cargo hold ladder while repairing a section of its hand railing. Bulker grounds off Australia A coal carrier that ran aground off Gladstone in central Queensland last week is still anchored off the harbour.The Panamanian-registered ship became stuck on a mud bank in the main shipping channel on Friday afternoon.Marine surveyors and divers have been inspecting the coal carrier.A spokesman from Marine Safety Queensland says it is waiting on Federal Government approvals before the ship is moved.The Australian Marine Safety Authority and Australian Transport Safety Bureau are compiling reports into the incident.Source: ABC News
