TAIC issued an investigation report on the sinking of the fishing vessel 'Francie' in inclement weather off Kaipara Harbour in November 2016, which killed eight people. TAIC highlighted that the eight passengers would have better chances of survival, had they wearing life-jackets in accordance with maritime safety rules. 

IMCA informed of an incident when a trainee Electrical Engineer drank from a mineral water bottle in the engine room workshop, which contained thinners rather than water. ETO realized that it was thinners and spat out most of the contents. He then drank salt water to induce vomiting in order to get any thinner out of his body. 

According to IMCA, on 14 December 2015, an agency worker was unchaining a vehicle ramp from a delivery lorry. The lorry then moved forward with one chain still attached to the ramp, crushing the worker between the ramp and a barrier. The company pleaded guilty, it has been fined £373,000 and ordered to pay costs of £8,333.

In its latest Safety Digest, UK MAIB describes an incident where a foreman fell from through a hatch and down two decks. The incident happened as the crewmembers were trying to lift a test skid. The foreman landed on his right, and was immediately restrained from the other crew in order not to risk further injury. He was later transferred to a hospital.

On 10 October 2017, the Netherlands-flagged general cargo vessel 'Ruyter' ran aground off Rathlin Island when the master, who was the watchkeeper, left the bridge unattended. The UK MAIB issued an investigation report on the incident, noting that the master had been consuming alcohol prior to taking over the watch.

TSB Canada issued an investigation report on the capsizing and sinking of the fishing vessel 'Miss Cory' in the Strait of Georgia, British Columbia, in March 2017, indicating inadequate vessel stability information as the key cause. One crew member was reported missing as a result from the accident. 

On 3 May 2017, the USCG response boat 'CG 29113' allided with the Highway 11 Bridge while responding to a non-distress search and rescue case involving an adrift sailboat in Lake Pontchartrain, Louisiana. The accident caused a minor injury to one USCG crew member and damage estimated at $337,000 to the CG 29113.

The NTSB issued an investigation report on the grounding and sinking of the fishing vessel 'Southern Bell' in the Gulf of Mexico, east of Sabine Pass Jetty Channel, in October 2017, that resulted in a release of approximately 3,800 gallons of diesel and oil into the water and in vessel total loss.

ATSB published a report about the collision between the container ship 'Glasgow Express' and the fishing vessel 'Mako.' The incident took place on 12 August 2017, 15 nautical miles south of Cape Woolamai, Victoria. The ATSB reported that a proper lookout by 'all available means' was not being maintained on neither vessel.

On 23 June 2015, in daylight with clear visibility, 'Jag Arnav' and 'Total Response' collided about 26 NM north-west of Bunbury, Western Australia. Jag Arnav sustained minor damage and no injuries to the crew. The ATSB investigation identified the poor lookout and lack of alertness as key causes of the accident.
