In September 2016, the ro-ro passenger ferry Hebrides was approaching Lochmaddy, Scotland when control of the ferry’s port controllable pitch propeller was lost. The master attempted to control movements, but he was unable to prevent it from running over several mooring pontoons and grounding.

On August 2016, the semi-submersible rig Transocean Winner grounded on the Isle of Lewis, following the loss of tow from the tug ALP Forward. The tug and tow was on passage from Stavanger, Norway to Valletta, Malta, when it encountered severe weather west of the Hebrides.

In November 2016, the owner of the 7.75m motor cruiser Vasquez fell unconscious after being overcome by carbon monoxide that had been emitted from failed rubber bellows that formed part of the inboard engine’s wet-exhaust system. Although rescuers came to his aid, it was not possible to save his life.

The UK MAIB has published its annual report including an overview of all accidents reported in 2016 along with details of their investigations and marine accident statistics. Last year has been a 'typically busy year', the UK MAIB says; there were 1,190 accidents reported and 29 investigations were started.

The UK MAIB issued an investigation report on the foundering of the fishing vessel Louisa SY30, while it was anchored of the Isle of Mingulay, on April 2016, resulting in three fatalities. The report provides important safety recommendations to prevent similar accidents from occurring in the future.

The tug Domingue girted and capsized while assisting the container ship CMA CGM Simba departing from the port of Tulear, Madagascar, on September 2016. As a result, two of Domingue’s five crew died. UK MAIB published an investigation report on the accident, providing significant safety issues. 

UK MAIB presented a case of collision between a domestic passenger vessel and a tug, due to unexpected hydrodynamic interaction, while operating in confined waters. MAIB provides a description of what happened and important lessons learned to prevent similar incidents from occurring in the future.

In its Safety Digest, UK MAIB describes an incident of hull damage of a ro-ro ferry, highlighting the importance of having a passage planning, even when crews are familiar with a route, as well as of using correctly ECDIS as an aid to navigation.

On 3 August 2016, a deckhand on board the twin rig prawn trawler Sea Harvester was seriously injured, after being struck on the head by a steel pole, during the recovery of fishing gear that had become caught on an underwater obstruction, in the Firth of Clyde, Scotland. The UK MAIB published a report on the investigation of the accident.

A C/O was seriously injured on board due to caustic soda which splashed into his face, entering under the edge of his protective mask. If the PPE does not fit, is not suitable for purpose, or cannot be worn for some reason, stop the work until proper protection can be provided, UK MAIB says.
