Transport Malta issued an investigation report on the fire on container onboard the ship Filia T while en route to Singapore in November 2019. The report could not determine the exact cause of the fire but identifies either friction or self-heating of the cargo as probable causes.

Transport Malta MSIU issued an investigation report on a serious injury of a crew member while testing the rescue boat crane onboard the oil/chemical tanker Ruby-T, on 20 October 2019. The report found likely that the bosun was distracted from understanding the actual risks of his own actions, as he was most probably concerned about the damages to the wire.

The Maltese registered LPG tanker Seasurfer ran aground in the Port of Praia in October 2019, as the pilotage section of the passage plan contained no specific directions on manoeuvring with the use of tug/s at Praia, investigation by Transport Malta MSIU revealed. 

Transport Malta MSIU issued an investigation report into the fatality and serious injuries on board the Maltese-registered passenger/roro vessel Euroferry Malta, in October 2019, in heavy weather conditions. The investigation highlighted that the anchors were not secured, as this was not common practice for the ship in such voyages.

Transport Malta reiterated the need of compliance with the Directive 2014/90/EU on Marine Equipment (MED). The Directive, inter alia, provides that recognised organisations shall inform the Directorate in cases where it is suspected that MED equipment placed onboard a Maltese ship is not compliant with the applicable requirements.

In light of a previous incident investigation conducted by Transport Malta, it is reminded that the shipping stakeholders should be cautious with the potential hazards associated with carriage of cargoes in bulk.

Transport Malta reminds of the limiting the sulphur content of marine fuels to 0.1% whilst operating in emission control areas, and the global sulphur limit of 0.5%, highlighting that several coastal States have implemented stricter regulations.

Transport Malta's Marine Safety Investigation Unit (MSIU) issued an investigation report on the collision of the Maltese registered oil tanker KANALA and the Chinese registered fishing vessel LIAODANYU 23626 on the Bohai Sea, China, in September 2019. 

Following three incident investigations, Transport Malta issued a notice to draw attention to the dangers of sheathed steel wires onboard ships due to restricted access to the wire rope for a thorough inspection and the possible factors for corrosion in wire ropes. 

Transport Malta MSIU issued an investigation report on the allision of the bulk carrier MV KLIMA with the Marshall Island-flagged bulk carrier SPRING SNOW and the Singapore-registered bunker barge LIBRA in the Eastern Special Purpose Anchorage, Singapore, in July 2019.
