Transport Malta MSIU issued an investigation report on a serious injury to a crew member onboard the tugboat Spinola while replacing a rescue boat cover at Timber Wharf, in April 2020. The investigation highlighted that a risk assessment was not considered necessary for this task.

Transport Malta MSIU issued an investigation report on the cargo explosion onboard the container ship MV Croatia while underway, in February 2020. The investigation established that flammable vapours had accumulated inside the container, which contained scrap metal and used car parts, resulting in an explosive atmosphere.

The investigation highlighted that there was no immediate barrier system around the opening in the deck, which could have prevented the fall. 

Transport Malta's MSIU issued an investigation report on two man overboards from the bulk carrier MV Stara Planina, in January 2020. The two crew members were lost overboard when unexpected waves washed over the poop deck, as they were not secured to the vessel.

Transport Malta issued an investigation report on a serious injury to a crew member in the grey water tank of MV LA SOLOGNAIS while underway in December 2019. The report revealed that a wiper, who entered the vessel’s grey water tank, suffered from the effects of possible hydrogen sulphide inhalation.

Transport Malta issued an investigation report on the engine-room fire onboard the Maltese-registered RoRo cargo vessel Eurocargo Trieste, following departure from the port of Livorno in November 2019. The investigation identified an accidental fuel spill onto a hot surface, as the most likely cause of the incident. 

Transport Malta issued an investigation report on the fire on container onboard the ship Filia T while en route to Singapore in November 2019. The report could not determine the exact cause of the fire but identifies either friction or self-heating of the cargo as probable causes.

Transport Malta MSIU issued an investigation report on a serious injury of a crew member while testing the rescue boat crane onboard the oil/chemical tanker Ruby-T, on 20 October 2019. The report found likely that the bosun was distracted from understanding the actual risks of his own actions, as he was most probably concerned about the damages to the wire.
