Transport Malta issued an investigation report on the serious injury of a crew member during mooring operations in the port of Marsaxlokk, Malta, in May 2020. The accident occurred when one of the stern lines parted and struck the third officer on the poop deck. 

Transport Malta issued an investigation report on the serious injury of a crew member onboard the tanker FT Furla, during mooring operations in Westlius Lock, Terneuzen, in May 2020. Analysis revealed that the mooring rope was in good condition, but its breaking load was markedly less than the certified breaking strength.

The investigation established that flammable vapours had accumulated inside the container, which contained scrap metal and used car parts, resulting in an explosive atmosphere.

Transport Malta MSIU issued an investigation report on the serious injury of a crew member during tank cleaning operations, onboard the Sichem Manila, while at Tarragona anchorage, Spain. The crew member was spilled with hot water while shutting off a valve.

Transport Malta MSIU issued an investigation report on the serious injury of a fitter while operating the lathe onboard the vehicle carrier MV Titania, in February 2020. The report found that a rod slipped from the tailstock while the lathe was rotating at a very high speed.

Transport Malta issued an investigation report on a fall inside a Water Ballast Tank of the Maltese-registered heavy lift/pipelay vessel Pioneering Spirit, in January 2020. The investigation highlighted that there was no immediate barrier system around the opening in the deck, which could have prevented the fall. 

Transport Malta's MSIU issued an investigation report on two man overboards from the bulk carrier MV Stara Planina, in January 2020. The two crew members were lost overboard when unexpected waves washed over the poop deck, as they were not secured to the vessel.

Transport Malta issued an investigation report on a serious injury to a crew member in the grey water tank of MV LA SOLOGNAIS while underway in December 2019. The report revealed that a wiper, who entered the vessel’s grey water tank, suffered from the effects of possible hydrogen sulphide inhalation.

Transport Malta issued an investigation report on the engine-room fire onboard the Maltese-registered RoRo cargo vessel Eurocargo Trieste, following departure from the port of Livorno in November 2019. The investigation identified an accidental fuel spill onto a hot surface, as the most likely cause of the incident.
