Transport Malta released the report on the explosion onboard "Seascout", on 18 March 2021, highlighting the dangers of working with heat or spark generating tools near locations where flammable vapour or volatile material may be present.

Transport Malta issued an investigation report on the grounding of the oil/chemical tanker MT KEY FIGHTER while underway in July 2020. The investigation revealed that the grounding was caused by the vessel’s deviation from the planned route because of loss of directional control.

Transport Malta investigated the fatal fall from height of a crew member in an enclosed space onboard the bulk carrier MV SEAFARER. Following the accident, the company reviewed the enclosed space access arrangements and issued a fleet safety information bulletin to all vessels.

Transport Malta issued an investigation report on the fire on a lifeboat while carrying out tests onboard the Maltese-flagged tanker Med Tuncer in May 2020. The investigation identified a very high short circuit current within the batteries as a probable cause of the incident.

Analysis revealed that the mooring rope was in good condition, but its breaking load was markedly less than the certified breaking strength.

Transport Malta MSIU issued an investigation report on a serious injury to a crew member onboard the tugboat Spinola while replacing a rescue boat cover at Timber Wharf, in April 2020. The investigation highlighted that a risk assessment was not considered necessary for this task.
