Transport Malta has published an investigation report regarding an incident that took place in the morning of 26 September 2022, whilst Milagro was drifting outside the Republic of Korea’s territorial waters, the fitter was assigned to replace a leaking hydraulic pipe on the main deck.

Transport Malta has published an investigation report regarding an incident that took place on 14 September 2022, when MV Mona was transiting the Aegean Sea, en route to a dry dock in Tuzla, Türkiye, when a fire broke out in the engine-room, with flames observed rising above the main engine’s cylinder unit no. 2.

Transport Malta has issued a revised  revised Merchant Shipping Notice to reflect the adoption of the Seafarer Portal as the sole direct link to apply for seafarer documentation. Furthermore, the  notice updates certain requirements that were introduced through the 2010 amendments to the STCW Convention (Manila Amendments).

Transport Malta issued an investigation report on the serious injury of a crew member onboard the container ship APL California, while letting go the towline at the port of Ningbo, China, in May 2020. The incident occurred as the bosun attempted to undo the knot connecting the tug’s pick-up gear and the vessel’s messenger line. 

Transport Malta investigated the fatal fall from height of a crew member in an enclosed space onboard the bulk carrier MV SEAFARER. Following the accident, the company reviewed the enclosed space access arrangements and issued a fleet safety information bulletin to all vessels.

Transport Malta issued an investigation report on the fire on a lifeboat while carrying out tests onboard the Maltese-flagged tanker Med Tuncer in May 2020. The investigation identified a very high short circuit current within the batteries as a probable cause of the incident.
