On 5 November 2016, an able seaman on board the Maltese registered roll-on rolloff ship Catherine was severely injured during cargo operations. He was discharged from the ship and was transferred to a local hospital for further treatment.

On 22 November 2016, the Maltese registered bulk carrier Robusto ran aground and remained stranded in the entrance of El Dekheila Channel, Egypt. Transport Malta's MSIU issued an investigation report, revealing that the course recommended by the pilot was different from the one prepared in the passage planning.

A bosun onboard the general cargo ship 'Garip Baba' lost his balance and fell to the poop deck, while carrying out maintenance routine tasks, injuring his back. He was transferred to a hospital and had to undergo surgical procedure. The Marine Safety Investigation Unit issued an investigation report on the incident.

MSIU issued an investigation report on the fatality of a crew member on board the Maltese registered container ship Sydney Trader, off Port Limits Anchorage, Hong Kong, in September 2016. The report provides a description of what happened and safety conclusions.

Transport Malta issued an investigation report, concerning the allision of the Antigua & Barbuda registered heavy-lift "MARIA" and the consequent foundering of the Italian registered fishing vessel "ANGELA ARCELLA", in the Grand Harbour, Valletta, on 9 August 2016.

On 17 June 2016, the passenger / car ferry Malita was involved in an accident while making fast at Quay No. 2, at the Ċirkewwa Terminal, Malta. It was reported that while Malita was still making fast at the quay, she went astern without pre-warning and at a time when the discharge of vehicles had already started.

On 19 June 2016, an engineer from the container vessel HS Rossini, was fatally injured when he fell on the deck of bunker barge Smit Bongani. The cause of the fall was neither related to the vessel’s operations nor to defects in the ladder, which was being used, Transport Malta says. However, the safety investigation analysed the situation from the perspective of missing barriers which would have otherwise prevented the fall to the barge.
