The US Cruise Ship National Center of Expertise cited a list of common causes of PSC detentions on cruise ships by the USCG throughout 2017. USCG conducted 292 cruise ship examinations in 2017 and only 0.34% received a detention, which shows there is a strong safety culture in the cruise line industry.

During an audit from 25 May to 1 June 2018, Norway's Petroleum Safety Authority found several irregularities on Repsol’s Gyda facility off Norway, linked to ignition source control, electrical installations, and work in and operation of electrical facilities.

A US Coast Guard cutter crew issued a violation to commercial fishermen who caught salmon outside of the fishing season 20 miles off the coast of Newport, Oregon, on Sunday. Commercial fishing for salmon is closed in the area between Cape Falcon and Humbug Mountain and does not re-open until 5 July.

The US Coast Guard terminated the voyage of the 35-foot pleasure craft 'Catviar', near Fisher Island, after two people from the vessel were rescued from the water by a good Samaritan, on Saturday. The owner and operator of the 'Catviar' faces maximum civil penalties amounting to $41,546 for this illegal passenger for hire operation.

The US Coast Guard ended the voyage of the vessel 'Pacific Provider' that was illegally carrying passengers for hire operating out of Whittier. The USCG proceeded to terminate the voyage after it found that the ship was breaching the Vessel Passenger Service Act. Two passengers stated that they paid for a trip on the Pacific Provider.

The US Coast Guard terminated the voyage of the 65-foot pleasure yacht Zenith at Sea Isle Marina, as it discovered several safety issues and environmental concerns, which included the carriage of more paying passengers than they were certified to carry onboard the vessel. The boarding was conducted after the yacht allided with the Venetian Causeway Bridge earlier that day.

The Captain of a Russian oil tanker was fined a total of £25761.99 at Hull Magistrates Court on 14 June, after breaching the International Safety Management Code. The prosecution was brought by the UK Maritime & Coastguard Agency. Vitaliy Trofimov, Captain of the Russian tanker, pleaded guilty to non-compliance of safety requirements.

In the short educational video in cooperation with Standard P&I Club, CHIRP Maritime analyzes a very good example of non compliance with collision avoidance regulations. CHIRP comments that, in open waters, there is absolutely no need for vessels to be in close proximity to one another.

Eleven people died following collision of a catamaran with a cargo barge in the Volga river, about 250 meters from the riverbank, on Monday evening. Five people were rescued, three of whom were hospitalized. Violation of catamaran maneuvering rules due to intoxication is considered the key cause of the accident.

Norway's Petroleum Safety Authority has completed its investigation of the fatal accident onboard the drilling rig 'Maersk Interceptor', on 7 December 2017, where one person was killed and another seriously injured. The investigation revealed several breaches of safety regulations.
