Safety culture and safety climate are similar terms describing an organization’s approach to safety, not only the visible efforts or attitudes its members chose to adopt, but also every mental function that defines their overall behavior. In other words, the climate is an item which is easily perceived by others while culture is the basis that lies hidden under the surface.

Following reports of a number of instances where defective pilot ladders were identified on foreign ships visiting British ports and the regular use of dangerously weighted heaving lines by some vessels at UK ports, the British Ports Association (BPA) has called on the international shipping community to face up to these ongoing safety issues.

Maritime NZ, New Zealand's shipping safety agency, released drowning statistics related to recreational boating, noting a significant progress with only four boating fatalities in 2018 compared to previous years. The agency provided simple advice for boaties to help keep that number down.

From 15 to 19 October 2018, PSA conducted an audit to follow up how Eni is complying with technical, operational and organisational regulatory requirements for electrical facilities on the Goliat FPSO in the Barents Sea. The audit had found regulatory breaches regarding managing risk and barriers, with the PSA giving notice of a order.

Discussing how to move forward with developing possible mandatory and recommendatory measures for ships operating in polar waters which are not currently covered by the Polar Code, IMO's MSC 100 agreed on a roadmap which could see revisions to SOLAS and/or the Polar Code considered for adoption in 2022.

IMO's MSC 100 approved revised guidelines on fatigue, which provide comprehensive information on the causes and consequences of fatigue, and the risks it poses to the safety and health of seafarers, operational safety, security and protection of the marine environment. The aim is to assist all stakeholders to contribute to the mitigation and management of fatigue.

 MV 'Tian Fu' , a Hong Kong flagged vessel, saved Susie Goodall, a British Skipper of 'DHL Starlight', after 50 hours vigil on December 7th. Susie sent out EPIRB distress alert from her vessel while competing in the Golden Globe Yacht Race.

As part of its safety investigation on the sinking of the scallop dredger 'Solstice' with one fatality in September 2017, the UK MAIB issued a safety flyer for the fishing industry to share lessons learned from the casualty which raised poor stability concerns for fishing vessels. 

UK MAIB released its report on the SMN Explorer accident. Namely, on 1 February 2018, a crewman from the cargo vessel SMN Explorer was fatally crushed as he was working on deck. Namely, a stowage space hatch cover fell on him. The investigation concluded that the ship’s safety management system was immature and the safety culture on board the vessel was weak.

An Officer of the Watch is responsible for keeping watch on the bridge to ensure the safe navigation of the ship based on a pre-defined Passage plan as approved by Master.  In this regard, the decision making process is vital during his/her watch as the final decisions reflect directly on the safety of the vessel.
