The USCG released the US Port State Control Annual Report for 2017, revealing an annual detention rate of 0.99%, the lowest ever in the US. However, the report showed a rise in detentions related to fire fighting, for the fourth straight year. Similarly, MARPOL Annex I deficiencies, which had been on the decline over the past several years, rose slightly in 2017. 

St. Vincent and the Grenadines released a circular regarding Port State Control Inspections with deficiencies which do not warrant detention, saying that ships or companies which do not comply with this circular will be subject to fines and the vessels may be deleted at short notice.

In its monthly safety scenario for January, the Swedish P&I Club describes a collision of vessels caused by insufficient combination of VHF communication and a proper maritime traffic look-out by sight and hearing.

The US Coast Guard released its Final Action Memo regarding the 2015 S.S. El Faro sinking, which resulted in the loss of its entire crew. In the Final Action Memo, USCG approves the findings of fact, analysis, and conclusions detailed in the Marine Board of Investigation’s Report of Investigation.

In its monthly safety scenario for December, the Swedish Club describes a case of a cargo ship listing due to overloading, as well as early deballast, that caused an AB to fall overboard and go missing until today. 

El Faro sinking, the deadliest shipping disaster involving a US-flagged vessel in more than 30 years, was caused by a captain’s failure to avoid sailing into a hurricane, despite numerous opportunities to route a course away from hazardous weather, the NTSB announced during a public meeting Tuesday

In October 2016, the articulated tug and barge Nathan E Stewart/DBL 55 ran aground on Edge Reef off Athlone Island, Canada, causing significant environmental damage, as 29,000 gallons of oil were released. Damage to the vessel and barge was estimated at $12 million.

UKHO published a revised edition to help bridge crews record and manage cyber risks. The update was developed in accordance with IMO's guidance, stating that approved SMS should consider cyber risk management in line with the ISM code before January 2021.

On 5 November 2016, an able seaman on board the Maltese registered roll-on rolloff ship Catherine was severely injured during cargo operations. He was discharged from the ship and was transferred to a local hospital for further treatment.

The UK’s fishing federations in conjunction with the MCA Fishing Safety ​team have developed a Fishing Safety Management System that will assist fishermen with the imminent implementation of a generational change to fishing safety legislation, the ILO 188 Fishing Convention.
