The latest monthly safety scenario by Swedish Club, describes an incident during which a bulk carrier grounded due to restricted visibility.

The Swedish Club analyzes an incident of a collision of a container ship with a fishing vessel, while in restricted visibility. The container ship continued its voyage after the incident, which was a very bad decision by the Master, the Club stressed. 

In its Monthly Safety Scenario for August, the Swedish P&I Club presents a case of ships collision due to restricted visibility as a result from fog. The incident highlighted safety issues related to compliance with COLREGs, on lookout, safe speed and conditions on restricted visibility.

In its latest Safety Digest, UK MAIB describes a near-miss of a Ro-Ro vessel. The ship was trying to berth but it encountered problems because at that day there was fog and sunlight in the port, which resulted in a significant glare. As the ship was performing manoeuvres to berth, the master warned the passengers and the crew to prepare for impact, which was averted, but for only 3 metres.

Midway through the watch, the OOW permitted the AB to perform cleaning and housekeeping duties, therefore becoming the sole lookout.
