In its monthly safety scenario, the Swedish P&I Club describes an incident of a crane fall onboard a container ship, which hit and injured a crew member. The investigation revealed that the wire was not in good condition, despite proper maintenance as per PMS system. 

The Swedish P&I Club has issued its Monthly Safety Scenario for August, regarding a charcoal incident of a container cargo vessel due to incorrect declaration. The IMDG Code for charcoal, if it applies, requires adequate heat treatment and then cooling of the charcoal before packing.

The Swedish P&I Club has issued its Monthly Safety Scenario for April, regarding a collision incident of a general cargo vessel with another vessel during overtaking. At the time of the incident, the ship was handled by the second officer.

The Swedish P&I Club has issued its Monthly Safety Scenario for March, regarding a grounding incident that happened due to unsufficient operations undergone when a vessel was decided to enter the Canadian waters.

The Swedish P&I Club has issued its Monthly Safety Scenario for February, regarding a series of explosions that occurred onboard a bulk carrier mainly due to the fact that a few fumigation pellets were not distributed as per the cargo document.

The Swedish P&I Club has issued its Monthly Safety Scenario for January, regarding an incident of fire onboard a RoRo vessel. However in this case the crew fought the fire and prevented the vessel from becoming a total loss.

The Swedish P&I Club has issued its Monthly Safety Scenario for December, regarding an incident in which the OOW of a vessel that was berthed alongside a quay, waiting to proceed through a lock to another berth, assumed everything should be OK for departure without having checked that the controllable pitch propeller (CPP) was not responding correctly and subsequently the vessel rapidly approached the lock.
