As UK MAIB presents in its latest Safety Digest, three fishermen went to haul some static nets. Yet, after sometime the vessel began sinking and the fishermen had to urgently call the coastguard to come to the rescue. Later on, the inspection showed that some equipment was outdated and wasn't working, despite the fact that the vessel had been updated by one of the fishermen five years ago. 

In the latest Safety Digest, the UK MAIB published a maritime incident, according to which a skipper and its fellow crew decided to anchore after many days of fishing. Yet, the vessel started sinking rapidly. A possible factor that caused the incident is fatigue, as all members of the vessel worked long hours.

UK MAIB, in the latest Safety Digest, addresses a situation where a potter was returning to harbour, when the sea conditions became rough. Eventually, the vessel sank and the crew fell in the sea waiting for Coast Guard's rescue. 

In its latest Safety Digest, UK MAIB focuses on a fishing vessel that dismissed the warning signs of submarine cables and fished in that area. This resulted to the vessel being tangled with the cables and then resulting to damage to UK infrastructure. 

According to Reuters, the Spanish Coast Guard saved Wladimir Klitschko, former heavyweight champion, on Sunday, June 23. Based on his tweet, his yacht caught fire resulting to the trip ending and his family and friends being evacuated by a rescue team.

As local sources report, a Filipino fishing boat, that had 22 fishermen onboard, sank after being hit by an alleged Chinese vessel, in Reed Bank, in the South China Sea. Since the area of the incident is in the middle of disputes, the accident increased the already existent tension between the countries.

The trend of the increasing size of ships creates more safety, as the frequency of shipping losses overall has steadily declined over the past decade. However, the cost of incidents has been increasing, driven in large part by the cost of claims involving large vessels, Allianz said in ts recent Shipping and Safety Review for 2019. 

As sources report, 17 people have confirmed dead and at least one remains missing after the overloaded Arim Jaya ferry was capsized off the coast of Giliyang Island, East Java, Indonesia. The victims include four children. Officials report that that 39 passengers survived.

In the latest Safety Digest, UK MAIB focuses on an explosion accident, according to which the master of a cargo vessel received unambiguous loading instructions that led to two explosions in the forecastle store. A crewmember that was in the room with the explosions suffered severe burns, whereas the investigation showed that the cargo was not in the IMSBC Code.
