Swedish Club casebook of maritime accidents describes a case where a container vessel ran aground due to poor cooperation.

Transport Malta issued an investigation report on the fire on a lifeboat while carrying out tests onboard the Maltese-flagged tanker Med Tuncer in May 2020. The investigation identified a very high short circuit current within the batteries as a probable cause of the incident. 

ATSB issued an investigation report on the fire onboard the self-unloading bulk carrier 'Iron Chieftain' at Port Kembla, in June 2018, which resulted from a failed bearing in the ship’s conveyor system. The investigation highlighted the inadequacy of fire safety regulations and standards for the cargo handling spaces on such ship types.

Following Swedish Club's casebook, it is described a grounding case of a suezmax oil tanker, while transiting the Suez Canal.

In a recent publication examining the most regular causes of engine damage onboard, the Swedish Club describes a case of lube oil contamination caused by forgotten rubber membrane seals in the engine room of a chemical/oil tanker. 

The Australian Transport Safety Bureau (ATSB) issued a preliminary report from its ongoing investigation into a fire onboard the multirole vessel MPV Everest while on charter to the Australian Antarctic Division, last April. 

In its latest released casebook of marine casualties, Swedish Club focuses on a bulk carrier grounding due to heavy weather conditions.

DMAIB issued an investigation report on the collision of the general cargo ship ICE ROSE with anti-submarine ship 311 KAZANETS of the Russian Navy in the Sound, Denmark, in September 2020. The investigation established that the collision happened as a result of the navigational practises on both ships on that day.

In its recently released casebook of maritime incidents, Swedish Club describes a grounding case, as the officer on watch missed the waypoint.
